Notice of Medical Records Retention and Destruction Practices
In accordance with 18VA85-20-26, Fairfax Pediatric
Associates, P.C., retains medical records for a minimum period of six
years from the last patient encounter. Medical records of minor
children, including immunizations, are maintained until the child
reaches the age of 18 or becomes emancipated, with a minimum time for
record retention of six years from the last patient encounter regardless
of the age of the child. In order to protect patient confidentiality,
purged medical records are destroyed by shredding.
