During your well check up, please bring any forms that need to be filled out for school, camp, sports etc. We will fill out the form during the visit and return it to you at the end of the visit. During other times, please leave the form at the front desk. It usually takes 3-5 business days to prepare the form for you. We will call and let you know when the forms are ready.

There is no charge for forms submitted at time of the child’s well check appointment. Please plan ahead and have your forms ready at the well check.

• $15 per form (outside of well child exam) collected at time of submission. 
• $30 for “Rush Service” form (completed with one business day) collected at time of submission.

Please note that the front staff does not have the authority to alter, reduce or waive charges. Fees are collected at time of form pick-up.
